Expert tips for choosing a plan

How we recognize whether a plan fits a self-employed career.

01

A plan must work in two directions

Expand benefits later when the business grows, and manage costs within the insurer's range of plans when business is weaker. A plan should not be a dead end.

02

With an option right, the conditions count

Whether an option right exists is the easy question. What matters are deadlines, age limits, target plans and which extra benefits you can reach without new health questions.

03

Rehabilitation belongs explicitly in the plan check

An entitlement from the Deutsche Rentenversicherung is helpful. It does not replace the question of how rehabilitation and follow-up rehabilitation are covered in the plan itself.

04

Krankentagegeld must be able to grow with you

If you start a business today and earn much more in five years, you should be able to raise your income protection instead of staying at the level of the start-up phase.

The selection criteria in detail

Option rights create room for the development of your business

A self-employed person can consciously choose a solid mid-range plan today and want higher benefits a few years later. By then, new diagnoses may have been added. Without a contractual option right, expanding the benefits later can become more difficult.

Alongside the statutory right to change plan under § 204 (1) VVG, contractual option rights are therefore relevant. § 204 VVG allows a switch to other plans of the same insurer with similar cover. For additional benefits, the insurer may require conditions related to your health.

With a good option right, we check more than whether it exists. For later use, deadlines, age limits, target plans and the question of which extra benefits you can actually reach without new health questions are decisive.

Check pointWhat we look for
TimingWhen can the option first be used?
RepetitionCan the option be used more than once?
Age limitUp to what age does the right apply?
Target plansWhich plans with stronger benefits are open to you?
Health questionsWhich extra benefits can you reach without a new check?
OccasionsDo certain life events trigger additional options?

For the self-employed, the route in the other direction also matters. If the business is weaker for a few years, an internal plan change or a different deductible can reduce the premium. A plan should therefore not be a dead end.

Rehabilitation benefits are a real selection criterion for the self-employed

When choosing a plan, we explicitly check how medical rehabilitation and follow-up rehabilitation are covered. A self-employed person does not automatically lose a possible entitlement to rehabilitation from the Deutsche Rentenversicherung just because they no longer pay regular pension insurance contributions.

For medical rehabilitation, the Deutsche Rentenversicherung names a minimum insurance period of 5 or 15 years, depending on the benefit. In other cases, at least six calendar months with compulsory contributions within the last two years can be enough.

If a self-employed person has already completed the relevant qualifying period of the pension insurance, the insurance requirements can therefore still be met, even though no contributions have been paid in recent years. Whether there is an entitlement in a specific case is decided by the Deutsche Rentenversicherung.

In retirement, responsibility changes. If you receive or have applied for an old-age pension of at least two thirds of the full pension, you generally no longer receive regular medical rehabilitation from the Deutsche Rentenversicherung. Oncological rehabilitation remains possible as an exception.

An existing entitlement from the Deutsche Rentenversicherung is helpful. When you choose private health insurance, however, it does not replace checking how rehabilitation and follow-up rehabilitation are covered in the plan itself.

Krankentagegeld is part of the best private health insurance for the self-employed

Self-employed people do not receive six weeks of continued pay from an employer when they are unable to work. That is why we always look at private health insurance for the self-employed together with suitable income protection.

In our advice practice 2026, around 99% of our self-employed clients choose Krankentagegeld starting from day 43, because for these clients this model offers the best balance between the ongoing premium and cover for longer loss of earnings (PKV-Welt advice practice 2026).

In 2026 our sample client insures €120 of Krankentagegeld  per day from day 43. The premium for this is €43.08 per month (PKV-Welt sample calculation 2026).

When choosing a plan, we also check how Krankentagegeld  can be raised as income rises. A business owner whose profit later grows considerably should be able to adjust their income protection instead of staying permanently at the level of the start-up phase.

Benefits and flexibility come before the lowest starting price

For us, a low monthly premium is not yet a sign of good private health insurance. For the self-employed, we mainly check benefits that can later become expensive or hard to replace.

Benefit areaWhy it matters for the choice
Inpatientaccommodation, private medical treatment and choices in hospital
Rehabilitation and follow-up rehabilitationclear rules in the plan if other payers are not or no longer responsible
Medical aidsopen or sufficiently broad cover for larger claims
Psychotherapylevel of reimbursement and possible limits
Dentaltreatment, dentures and staggered limits in the first years

The deductible should not be looked at in isolation either. For our sample client, born in 1992, the same plan in 2026 including private Pflegepflichtversicherung PflegepflichtversicherungCompulsory long-term care insurance. Everyone with health insurance in Germany must have it, privately insured people through their private insurer. costs €535.69 with a deductible of €0, €439 with a deductible of €600 and €381.78 with a deductible of €1,200 (PKV-Welt sample calculation 2026).

The three variants show the financial room that different deductible levels can create. The detailed calculation is in the costs for the self-employed.

Our choice follows four criteria

We do not create a general ranking from first to fifth place. For the self-employed, a plan must meet four requirements at the same time.

CriterionOur check
BenefitsThe contractual cover must convince in the relevant benefit areas.
OptionLater improvements should be possible under clear conditions without new health questions.
FlexibilityIf business is weaker, there must be internal levers.
Krankentagegeld The income protection must fit your actual self-employment.

For many self-employed people, a solid mid-range plan with good option rights can therefore be more interesting than a particularly cheap entry plan or cover that is fully equipped from the start. Which criteria apply regardless of occupation is covered in our article on the best private health insurance.

Before the application, we check anonymously whether you can be accepted with your health history. From over 5,000 applications with health questions that we have handled, we know that insurers can assess the same medical history differently (PKV-Welt experience 2026).

The best private health insurance for the self-employed must therefore work in two directions. It must let you expand benefits later when the business grows, and manage costs within the insurer's range of plans when business is weaker. How the premium develops in old age also belongs in this assessment.

Switching to private health insurance is probably not advisable for you.

From the age of 55, admission to private health insurance is often possible only with restrictions. A late switch can also mean higher premiums.

It matters to us that a switch pays off for you in the long run. That is why we do not recommend a consultation when your answers already show that the conditions are rather unfavorable.
