The key points about health insurance after marriage at a glance.

01

Your insurance status does not change automatically

At first, marriage leaves everything as it was. If you have statutory health insurance, you stay in statutory health insurance. If you have private health insurance, you keep your existing private contract. A change only happens if you actively apply for it and meet the requirements.

02

Duty to report your marital status

Many insurance terms state that changes in marital status must be reported to the insurer. In practice, a short letter with your contract number and a copy of the marriage certificate is usually enough. This keeps your contract data up to date and helps you avoid questions later.

03

Check family insurance in statutory health insurance

If one spouse has statutory health insurance, the other spouse can, under certain conditions, be covered free of charge through family insurance. One key condition is that their own regular income does not exceed the permitted limit. For 2026, this limit is generally €565 per month, or €603 per month with a mini-job. From 2027, according to a cabinet decision, it will be €590, or €633 with a mini-job.

04

Marriage alone gives no right to switch to private health insurance

Marriage itself does not automatically give you a right to be accepted into your spouse's private health insurance. A switch to private health insurance can still be possible if you meet the personal requirements, for example as a civil servant, as a self-employed person or as an employee above the Jahresarbeitsentgeltgrenze. In 2026 it is €77,400 per year.

05

Think about family planning early

After the wedding, you should also clarify early how future children can be insured. Whether a child can be covered free of charge through family insurance in statutory health insurance or needs a private contract of its own depends on factors such as the parents' insurance status and income. This point can become decisive later, especially if one parent is privately insured and the other is in statutory health insurance.

Typical situations and common questions

What applies if both spouses have statutory health insurance?

If both spouses are already insured in statutory health insurance, their insurance status at first remains unchanged. Each keeps their existing membership.

A review can still make sense if one spouse has no income of their own or only a low one. In this case, free family insurance through the spouse with statutory health insurance may be possible.

When is free family insurance possible?

If one spouse has statutory health insurance and the other has no regular income or only a low one, free family insurance in statutory health insurance can be possible.

For 2026, an income limit of €565 per month generally applies. With a mini-job, the limit is €603 per month. From 2027, the limits rise to €590 and €633. In addition, there must be no grounds for exclusion, such as being self-employed as your main occupation or a compulsory insurance of their own that takes priority.

This situation can be very attractive financially, because the covered spouse pays no health insurance contribution of their own.

What applies if one spouse is privately insured and the other has statutory health insurance?

In a mixed situation, too, everything at first stays the same after the wedding. The privately insured spouse stays in private health insurance, and the spouse with statutory health insurance stays in statutory health insurance.

Free family insurance for the spouse with statutory health insurance through the privately insured spouse does not exist. In private health insurance, each person is insured individually and pays a premium of their own.

Whether the spouse with statutory health insurance can switch to private health insurance does not depend on the marriage. It depends on the general requirements, for example professional status, income and the health questions.

What changes if both spouses are privately insured?

If both spouses are already privately insured, marriage generally changes nothing about the existing contracts. Both keep their own private contract with a premium of their own.

A joint plan review still makes sense. It should check whether the marital status has to be reported, whether there are special rules for spouses and how insuring children later would work.

For family planning, it is important to know that children are not automatically covered free of charge in private health insurance. For each child, you have to check whether family insurance in statutory health insurance is possible or a private contract of its own is needed.

What applies if one spouse is a civil servant and the other is an employee?

This situation should be checked especially carefully. The civil servant usually has their own entitlement to Beihilfe and insures only the remaining share of costs privately.

The employed spouse, however, does not automatically gain an entitlement of their own through the marriage. Whether a spouse can be included as an eligible dependent in the Beihilfe BeihilfeThe state healthcare allowance for civil servants. The Beihilfestelle pays a fixed share of medical costs, the Beihilfesatz, and private insurance covers the rest.More about Beihilfe → depends on the relevant rules and income limits. These differ between the federal government and the individual federal states.

For the employed spouse, it also remains decisive whether compulsory insurance in statutory health insurance applies or whether their income exempts them from it. In 2026, the general Jahresarbeitsentgeltgrenze  is €77,400 per year. Only when regular annual earnings exceed this limit can an employee generally choose between voluntary membership in statutory health insurance and private health insurance.

What do we recommend to couples after the wedding?

From our experience of over 5,000 applications with health questions, a recurring pattern emerges. Couples marry, the new marital status is not reported to the insurer, and years later questions arise in a claim because the contract data is no longer fully up to date.

That is why we recommend that you do not only report the marriage formally, but also use it as an occasion for a joint insurance review. The main points are income, professional status, family planning, possible Beihilfe  entitlements, insuring children and the long-term premium development.

For self-employed people in particular, the wedding can coincide with a more stable income or a general reorganization of private finances. This does not create an automatic right to switch to private health insurance, but it can be a sensible time to reassess existing plans and possible alternatives.

An early review can also make sense for younger married couples. In private health insurance, the entry age plays an important role. If you are generally eligible for a switch and are thinking about private health insurance in the long term, you should not wait until 40 to start checking your options.

Does the same apply to registered civil partnerships?

Since the introduction of same-sex marriage in 2017, registered civil partnerships and marriages have generally been treated equally in German social insurance law. Family insurance under § 10 SGB V is therefore also open to registered civil partners if the requirements are met. For private plans, the exact wording of the insurance terms is what counts.

What happens if a spouse's income falls below the Jahresarbeitsentgeltgrenze?

If the income of a privately insured employee falls below the Jahresarbeitsentgeltgrenze  of €77,400 (2026), compulsory insurance in statutory health insurance applies. Privately insured employees can then no longer stay in private health insurance if the employer applies the compulsory insurance and a return to statutory health insurance is due. This has nothing to do with the marriage itself, but life after the wedding sometimes changes in this direction, for example when someone moves to part-time work.

How long after the wedding does a special acceptance clause of the insurer apply?

This differs from plan to plan. Where a clause on adding a spouse exists, there is typically a deadline of two months from the wedding. After that, the special acceptance option lapses and the regular health questions apply. The exact deadline is in the insurer's insurance terms. It is best to ask before the wedding, not after.

Does marriage affect the monthly premium in private health insurance?

Not directly. Premiums in private health insurance depend on entry age, plan and state of health at entry, not on marital status. There can be indirect effects if the plan contains beneficiary designations or specific clauses on family members. You should look this up in the plan's own schedule of benefits.

What applies to a marriage abroad?

German health insurance recognizes the marital status from the date the marriage legally takes effect, regardless of whether the wedding took place in Germany or abroad. A marriage concluded abroad that is recognized under German law has the same insurance consequences as one concluded in Germany. The marriage certificate may need a certified translation.
