Important tips for tax advisors

A switch to private health insurance needs careful thought. The most important points before you decide.

01

Use your free access as a self-employed professional

As a self-employed tax advisor, you can switch to private health insurance regardless of your income. A Jahresarbeitsentgeltgrenze JAEG · JahresarbeitsentgeltgrenzeThe annual income threshold in Germany. Employees who earn more than this per year may leave statutory health insurance and switch to private health insurance.More about JAEG → as for employees does not apply to you.

02

A premium independent of income

Your premium does not rise with the profit of your practice. Unlike the voluntary statutory contribution, it stays stable even in strong years. This makes it predictable.

03

Match Krankentagegeld to your practice

Without an employer, nobody keeps paying your wages when you are ill. Take out Krankentagegeld KrankentagegeldPrivate sick pay: a daily allowance from private insurance while you are unable to work. It replaces lost income from an agreed day of illness.More about Krankentagegeld → that covers running costs and living costs. Many plans pay from day 15.

04

Use the tax advantage

Premiums for basic cover can be claimed as pension and insurance expenses. You know this from your work, but it is often overlooked in your own planning.

05

Versorgungswerk and statutory insurance in retirement can be expensive

In retirement, tax advisors are usually classed as voluntary members of statutory health insurance. This can be expensive, because as a voluntary member you must pay statutory contributions on all types of income (payments from the Versorgungswerk VersorgungswerkThe professional pension scheme of the chambers, for example for doctors, lawyers and tax advisors. Members pay into it instead of the statutory pension insurance.More about Versorgungswerk →, company pensions, investment income, rental income and so on).

06

Watch out for rehab and spa treatment

The Versorgungswerk  does not offer ideal cover for rehab and spa treatment. So always include this benefit in your private health insurance plan.

Real cases from our advice

Real situations that show how individual the right solution is.

Opening a practice at 35

A premium independent of income that stays stable even in strong years and makes planning easier.

Looking ahead to retirement

In statutory insurance, payments from the Versorgungswerk  often lead to the maximum contribution. Private health insurance stays independent of income.

Out of the practice

Krankentagegeld  covered the running costs of the practice and living costs during a longer illness.

Versorgungswerk: disadvantages in old age in statutory health insurance

The Versorgungswerk  plays a key role in the life of a tax advisor. As a special institution for the liberal professions, it makes sure that its members are well provided for in old age. When it comes to health insurance contributions in retirement, however, membership has a major disadvantage.

In retirement, you can be a member of statutory health insurance either as a voluntary member or as a compulsory member of the Krankenversicherung der Rentner KVdR · Krankenversicherung der RentnerStatutory health insurance for pensioners. Whether you qualify depends on how long you were in statutory insurance during your working life. (KVdR). Members of the KVdR pay contributions only on certain types of income, above all on the statutory pension. Rental and investment income stay free of contributions. One requirement for the KVdR is that the person was in statutory insurance for at least nine tenths of the second half of their working life and receives a statutory pension.

Because tax advisors pay into the Versorgungswerk and not into the statutory pension insurance, they are usually classed as voluntary members in retirement. Voluntary members pay health and long-term care insurance contributions on all types of income. Together with the often higher pension from the Versorgungswerk , this frequently leads to the maximum contribution of around €1,000 in retirement. You carry it alone, because Versorgungswerke pay no allowance toward health insurance contributions. Private health insurance, in contrast, stays independent of income.

Tax deductibility of private health insurance premiums

The costs of basic cover, which offers benefits comparable to statutory health insurance, are fully deductible, as is any Zusatzbeitrag ZusatzbeitragThe additional contribution each statutory health insurance fund charges on top of the general rate. Its level differs from fund to fund.More about Zusatzbeitrag →. It gets more complex with extra benefits above the statutory level, such as high-quality dentures or chief physician treatment. The split is set out in the regulation on determining health insurance contribution shares (KVBEVO).

Basic benefits (deductible)Comfort benefits
Outpatient basic cover: 54.60%Krankentagegeld : not deductible
Inpatient basic cover: 15.11%Daily hospital allowance: not deductible
Dental basic cover: 9.88%Alternative practitioner: 1.69%
Total basic: 79.59%Chief physician or two-bed room: 9.24%
Single room: 3.64%Dentures and implant treatment: 5.88%
Orthodontics: 0.26%

So always compare net premiums, not gross premiums.

Tax advisors: insider knowledge on private health insurance

Special acceptance options through group contracts

For tax advisors, there are DKV group solutions with special acceptance rules. Especially with existing illnesses, you should not assume too quickly that private health insurance is no longer possible.

PKV-Welt practical insight: With a difficult medical history, the way you reach the insurer often matters as much as the later choice of plan.

A special plan only for tax advisors

The DKV does not place tax advisors in a general Krankentagegeld plan. With the KGTS, there is a special plan of its own for this professional group. This shows why a pure price comparison often falls short for professions with a chamber, because the conditions specific to the profession also matter.

Running costs of the practice can be decisive

For self-employed tax advisors, the financial burden does not end when they become unable to work. Staff salaries, rent, IT and leasing keep running. In its special Krankentagegeld  plans, the DKV explicitly allows these running costs of the practice to be insured as well.

So the question is not only how much income you lose, but also which costs keep running despite your illness.

Krankentagegeld also without private health insurance

According to the DKV, the special Krankentagegeld  can also be taken out on its own, in addition to existing health insurance with another provider or to a statutory membership. The DKV states that it waives its ordinary right of termination for these stand-alone contracts as well.

How much does private health insurance cost for tax advisors?

For tax advisors, private health insurance starts at around €300. Since no employer pays half, you carry the premium alone. In return, it does not depend on your profit. Plans with good medical benefits cost about €450 to €600 per month. Krankentagegeld  comes on top, depending on the amount.

What monthly premiums should you expect in statutory and private health insurance?

Voluntary members of statutory health insurance pay according to income. With a higher profit, they quickly reach the maximum contribution of up to €1,261.32 per month including long-term care insurance, with no employer contribution. In private health insurance, solid plans start at around €450 per month. Krankentagegeld  is calculated on top.

How do premiums develop in the long term?

In the long term, you should expect rising premiums. From 2006 to 2026, premiums in private health insurance rose by an average of 3.4% per year according to the association of private health insurers, and in statutory health insurance by 3.9%. Since no employer cushions the premium, a solid calculation matters for tax advisors. This is why we pay special attention to premium stability.

How do costs stay manageable in old age?

The premium is calculated regardless of income. This helps when your work in the practice winds down. Ageing reserves cushion the premiums, and the statutory surcharge of 10% ends from age 60. With a premium relief plan, you lower your premium in retirement in a targeted way.

How can I reduce my premiums?

A deductible suits tax advisors with savings and lowers the premium. An internal plan change under § 204 VVG often saves money without changing insurer and without losing your ageing reserves. The amount of your Krankentagegeld  should also match your needs.

Which benefits matter for tax advisors?

Popular benefit modules you can choose in your private health insurance.

Dental treatment and dentures

Private health insurance should cover at least 80% of high costs for dental treatment, crowns, implants and other dentures.

Chief physician treatment and single room

Often a cost driver: how important are these benefits to you? This is a personal decision.

Preventive check-ups

Private health insurance should reimburse the important preventive check-ups without restrictions.

Therapeutic treatments

Physiotherapy, speech therapy and occupational therapy should be reimbursed without tight benefit limits. Daily work in a practice means a lot of sitting, and your back will thank you.

Rehab and spa treatment

The Versorgungswerk  does not offer ideal cover for rehab and spa treatment. Your private health insurance plan should therefore always include this benefit.

Medical aids

Private health insurance should pay for medically necessary aids such as hearing aids, bandages or orthopedic insoles.

Accommodation and optional benefits

Single or two-bed room and optional medical benefits, depending on the comfort you want in hospital.

Frequently asked questions

Can tax advisors always take out private health insurance?

Self-employed tax advisors have free access to private health insurance, regardless of income. Employed tax advisors can switch as soon as their gross salary is above the Versicherungspflichtgrenze .

Do tax advisors get an allowance toward the premium?

Self-employed tax advisors usually do not, because no employer is involved. In return, the premium does not depend on profit and is predictable. You can also claim the premiums for basic cover for tax purposes.

Why is Krankentagegeld important for tax advisors?

Without an employer, there is no continued payment of wages during illness. Krankentagegeld  replaces your lost income from an agreed day and covers the running costs of your practice and your living costs during a longer illness.
