Your employer pays up to 50% of your private health insurance premium, capped at the maximum contribution. In 2026 this is €508.59 per month. In 2027, according to the cabinet decision, it is expected to be €557.81. Depending on your situation, there is a further contribution for the private Pflegepflichtversicherung PflegepflichtversicherungCompulsory long-term care insurance. Everyone with health insurance in Germany must have it, privately insured people through their private insurer.. This often makes good benefits affordable. What matters is the fit of plan, deductible and long-term stability.
In everyday life, it is not only the premium that counts, but the reimbursement when you make a claim. Look for a clear process for submitting bills and answering queries, transparent rules on the required documents and a service you can reach when things get complicated.
Your situation can change, and the plan should be able to follow. Useful are an adjustable deductible, benefit modules you can add later and the option of an internal plan change if your priorities shift.
Self-employed people pay the full private health insurance premium themselves. Besides the scope of benefits, premium stability, predictability and cover for loss of income therefore matter most.

For the self-employed, Krankentagegeld KrankentagegeldPrivate sick pay: a daily allowance from private insurance while you are unable to work. It replaces lost income from an agreed day of illness.More about Krankentagegeld → is often the most important module, because during a longer illness your income stops while your running costs remain. Look at when it starts (waiting period of 29 or 43 days), a realistic amount for private and business costs and the option to adjust it later.
A suitable deductible makes the plan more predictable. The goal is not the highest possible amount, but one you can realistically carry even in a weak year, with a clear annual cap and the option to adjust it later.
When your income depends on your ability to work, fast treatment counts. Plans without a primary care doctor rule, with free choice of doctor and simple digital submission help to stop a single bill from turning into a never-ending task.

Beihilfe covers part of the costs, and you insure the rest privately. What matters is a Restkostentarif RestkostentarifA private plan that covers the share of medical costs that Beihilfe does not pay. Together with Beihilfe it gives civil servants full cover.More about Restkostentarif → that exactly matches your Beihilfesatz, plus a supplementary plan for typical gaps. Medical aids, dentures and optional hospital services are particularly relevant.
Do not only assess the starting premium, but also the development of recent years. Look for solid plan pools, sufficient reserves and comprehensible premium adjustments. For relief in old age, it is worth looking at separate relief plans.
Start with your must-have criteria and then compare the terms in detail. Outpatient, inpatient and dental cover with clear reimbursement rates are relevant, plus rehabilitation and spa treatment, because Beihilfe often sets limits here.
For doctors and medical professionals, the focus is on plans with strong benefits that also work well in everyday life. The table gives a first orientation on premium, level of benefits and plan quality.

Some insurers offer special plans for medical professionals. They can be attractive, but they are not automatically the best choice. It makes sense to compare in two directions: doctors' plans, if benefits and terms fit, and high-quality regular plans, which are often broader.
If you are covered by a Versorgungswerk VersorgungswerkThe professional pension scheme of the chambers, for example for doctors, lawyers and tax advisors. Members pay into it instead of the statutory pension insurance.More about Versorgungswerk →, you should plan your private health insurance for the long term. What matters is not only today's premium, but the burden in retirement, the past stability of the plan and options for relief in old age.
Rehabilitation and spa treatment are handled differently depending on the pension and care system. Make sure that inpatient and outpatient medical rehabilitation is clearly insured, check which spa benefits are included and which proof is required in advance.
Children need their own contract in private health insurance. A comparison that clearly separates benefits and terms is worth it, especially for dental care, orthodontics and hospital benefits.

For newborns there is the Kindernachversicherung KindernachversicherungAdding a newborn to a parent's private policy without health questions. The application must reach the insurer within two months of birth.More about Kindernachversicherung →, and cover starts without waiting periods. The application must be made within the first two months. One parent must already be privately insured, and the contract must have existed for some time (at least three months). The child's plan must not be better than the parent's plan.
For children, it pays to look at frequently used benefits: orthodontics with clear rules and good reimbursement, therapies such as physiotherapy, occupational therapy and speech therapy, and solid inpatient cover.
For children, a low deductible or none at all often makes sense, because there are many checkups and doctor's appointments in the first years. Choose the deductible so that it still feels right with frequent visits to the pediatrician.
The tables rate each plan by five criteria. Factors marked ×2 count double because in the long run they have the strongest effect on price and everyday use. The points are a guide based on our analysis. What counts is always the specific policy terms and your personal situation.
We show you what you should know before switching to private health insurance.
Premiums are individual and depend on age, state of health and the benefits you want, not on income. According to the Association of Private Health Insurers, they average around €529 per month, with a wide range. Younger and healthy people often start at a lower price. Civil servants pay significantly less on average because of Beihilfe . Between 2006 and 2026, premiums rose by an average of 3.4% per year, in statutory health insurance by 3.9%.
There is no single best plan, only the one that suits you. First clarify which benefits matter to you today and in the future, for example fertility treatment, level of care or dental benefits. Then similar plans from several insurers can be calculated and compared side by side. This shows the differences in premium, benefits and acceptance.
A good plan reimburses doctors' fees at least up to the standard maximum rate of the official fee schedule (2.3 times), better up to the maximum rate (3.5 times). Free choice of doctor and hospital also matter, as do high-quality dental cover with at least 75% for dentures and cover abroad including repatriation. Optional extras are glasses and contact lenses, alternative practitioners and a no-claims refund.
Today's premium applies to the first year of the contract. The development over ten years shows how the plan has behaved under real market conditions. Whether a plan pays off is decided by the premium over the entire period of insurance, not by the premium of the first year.
That is possible. The critical point is assessing acceptance. If you do not know how insurers accept applicants with different health profiles, you risk a formal application and, in the worst case, a registered rejection. Brokers also have experience of how the individual insurers reimburse bills when you make a claim.
Always, especially with pre-existing conditions or risk factors. The anonymous risk enquiry shows the real range of acceptance between insurers, without creating an entry in an information database.
