Confirmation of cover before therapy starts
We recommend that you submit the application to the insurer before therapy starts and wait for the written confirmation of cover. This secures reimbursement for the whole course of treatment.

How does the consultation work? →| Criterion | Statutory | Private |
|---|---|---|
| Therapy place | search for a therapist with a statutory health insurance license, often a wait of several months | access to private practices as well, usually much faster |
| Approval | application and in part an expert review, based on fixed session quotas | application under the plan, confirmation of cover from the insurer |
| Number of sessions | fixed quotas per type of therapy | depends on the plan, typically 25 to 50 sessions per application with an option to extend |
| Billing | billing via the health insurance card | bill to the patient, reimbursement by the insurer |
In private health insurance, the scope of psychotherapy is set by the plan, not by a statutory catalog. Common are 25 to 50 sessions per approved application. Once this quota is used up, the therapist applies for an extension with a medical reason. Some plans reimburse only part of the cost after a certain number of sessions, for example with an out-of-pocket share of twenty or thirty percent from the thirty-first session. This exact clause should be read before you sign the contract.
The recognized methods are reimbursable, among them behavioral therapy, psychodynamic and analytical psychotherapy and, in many plans, systemic therapy. Which form of therapy fits in the individual case is decided by the patient and the therapist, not by the insurer and not by this article.
The process follows a fixed order. First come the trial sessions with the licensed therapist, then the application with diagnosis and therapy plan to the insurer, then the written confirmation of cover and finally the actual treatment. In most plans, the trial sessions themselves are reimbursed without approval. Their explicit purpose is to clarify whether the therapy will go ahead.
Billing follows the fee schedule for doctors, which also applies to psychological psychotherapists. A session of fifty minutes costs between €100 and €180, depending on the multiplier. With fifty sessions, this adds up to treatment costs of several thousand euros, which explains why insurers insist on the application process. The amounts are guide values. The plan and the fee agreement are binding.
For applicants with experience of therapy, the main topic is not reimbursement but the health questions. With most insurers, ongoing or recent psychotherapy leads to the application being postponed or rejected. Depending on the insurer, completed treatment is asked about retroactively over a period of five to ten years. This is not a judgment of the therapy but risk calculation. From the insurer's point of view, what counts are possible follow-up costs, not the reason.
From more than 5,000 applications with health questions, we know that the range of decisions is wide. Some insurers accept a completed short-term therapy after a life crisis on standard terms, others with a surcharge. We strongly recommend that applicants with earlier psychotherapy make an anonymous risk enquiry before they apply. This shows the risks of exclusions and surcharges early, without a rejection on record.
The therapist applies for an extension with a medical reason. Many plans approve further quotas, in part with a reduced reimbursement rate. A hard stop at a fixed number of sessions is the exception. A new review of the medical need is the rule.
In principle yes. The special waiting period of eight months has no statutory exception for emergencies. In practice, some insurers are accommodating if a doctor confirms the urgency, and acute treatment by a doctor is covered from the first day anyway. If you need help urgently, contact the out-of-hours medical service or an acute outpatient clinic, regardless of any insurance questions.
As a rule, no. What is reimbursed is medically necessary treatment of a diagnosed illness. Couples counseling without a medical condition does not fall under this, not even in comfort plans. Systemic therapy with a diagnosis is a different matter. It is reimbursable in many plans.
Increasingly yes, provided the licensed therapist conducts them and the billing follows the fee rules. Pure coaching or app offers without a licensed practitioner, by contrast, are not medical treatment and are not reimbursed.

From the age of 55, admission to private health insurance is often possible only with restrictions. A late switch can also mean higher premiums.
It matters to us that a switch pays off for you in the long run. That is why we do not recommend a consultation when your answers already show that the conditions are rather unfavorable.
