Affiliated hospital bed: what inpatient treatment by an affiliated physician means

Not all hospitals have affiliated beds. Hospitals that offer the model sign contracts with specialists in private practice from the area. Beds are reserved for these doctors, which they can use for their patients. The system is particularly common in medium-sized hospitals that position themselves through close cooperation with doctors in private practice.

The affiliated physician actively brings their patient to the hospital. They were the treating doctor in the practice, they perform the procedure as an inpatient treatment, and they take care of the aftercare. For the patient, this means continuity: no handover meeting, no coordination between the practice and an unfamiliar hospital doctor, no need to explain the medical history again.

The hospital provides the hospital infrastructure, meaning operating room, anesthesia, nursing, laboratory and technical equipment, and charges a daily hospital rate for it. The medical part of the bill comes separately from the affiliated physician. This separation is important for privately insured people, because the two bills can be assessed differently. Inpatient stays in an affiliated hospital bed are typical for procedures in ophthalmology, gynecology, ENT and for smaller orthopedic operations, so often for planned procedures where the patient prefers a familiar specialist.

Private plans differ in how they assess treatment in an affiliated hospital bed. Some reimburse the affiliated physician's share in the same way as regular inpatient chief physician treatment, others require the affiliated physician to meet certain contractual conditions, or limit reimbursement to certain specialties. If you have high-quality inpatient cover, you usually benefit in an affiliated hospital bed as well. Plans with a good hospital clause reimburse the medical share under the fee schedule, the hospital costs at the daily hospital rate and, if agreed, also comfort optional hospital services such as a single room.

The affiliated hospital bed as such says nothing about the room standard. A patient in an affiliated hospital bed can be in a two-bed room or in a room with several beds. Many policyholders confuse the affiliated hospital bed with a comfort level for private patients, but that is wrong. Optional hospital services such as a single room or the right to choose your doctor freely in hospital are separate contractual agreements, which are taken out independently of the affiliated bed model.

Billing runs on two tracks: the affiliated physician issues a bill under the fee schedule, and the hospital issues a separate bill for accommodation and nursing. Both must be submitted to the private health insurer. If you forward only one, you receive only partial reimbursement. We recommend asking explicitly about the affiliated physician clause when you take out a plan, because it is a specific part of the contract and not a standard you can take for granted.
