How reimbursement works and where out-of-pocket shares arise for therapeutic remedies.

01

Prescription before treatment starts

What is reimbursed is therapy prescribed by a doctor. The prescription states the diagnosis, the therapeutic remedy and the number of treatments. Common are series of six to ten appointments, with a follow-up prescription if needed.

02

Know the maximum rate clause

Many plans reimburse therapeutic remedies up to fixed maximum rates per treatment, often based on the rates eligible for Beihilfe BeihilfeThe state healthcare allowance for civil servants. The Beihilfestelle pays a fixed share of medical costs, the Beihilfesatz, and private insurance covers the rest.More about Beihilfe →. Private practices set their prices freely and are often above them. We recommend that you compare the practice price with the plan clause before the series starts.

03

Accept no caps on the number of treatments

Good plans reimburse as many treatments as are medically prescribed. Plans with fixed annual limits for therapeutic remedies quickly become tight with chronic complaints. This belongs in the plan comparison.

04

Physiotherapy is not osteopathy

Prescribed physiotherapy is covered by the therapeutic remedies part of the plan, osteopathy by the component for naturopathic treatment. Even if both take place in the same practice, the bills belong to separate budgets of the plan.

Physiotherapy in private health insurance in detail

How does reimbursement of physiotherapy work?

The process is always the same. The doctor prescribes after a diagnosis, the physiotherapist treats you and issues a private bill, and the insurer reimburses according to the plan. Most plans do not require approval before therapy starts. Typical reasons range from back complaints and aftercare following operations to the treatment of chronic joint diseases.

TreatmentTypical private price
Exercise therapy (individual treatment)€25 to €40
Manual therapy€30 to €50
Manual lymphatic drainage€30 to €60, depending on duration
Classic massage€20 to €35

Physiotherapists are not bound by a state fee schedule and set their private prices themselves. The ranges in the table are guide values. In large cities, prices tend to be at the upper end.

What is the maximum rate trap and how do plans solve it?

The most common out-of-pocket share in physiotherapy does not come from missing cover but from the maximum rate clause. The plan reimburses a fixed amount per treatment for the prescribed therapeutic remedy, the practice charges more, and the patient pays the difference. For a single series these are manageable amounts. With chronic needs over years, they add up to a noticeable sum.

High-performance plans instead reimburse at the customary local or an appropriate level, or state generous rates. If you need physiotherapy regularly, give this clause more weight in the plan comparison than some more prominent components. In an existing contract, it helps to compare the practice price, the plan rate and the difference per series.

What do we recommend on physiotherapy in the plan?

From more than 5,000 applications with health questions, we see physiotherapy from two angles. It is the most common therapeutic remedy item in existing contracts and a routine topic in the health questions. Prescribed series for back complaints are among the most common answers of all. Depending on the reason and the course, insurers rate them anywhere from unproblematic to subject to a surcharge. Clean documentation that the treatment has been completed helps with every later application.

We recommend that you obtain a confirmation of cover before longer series of treatment and submit bills collected per prescription series. We also recommend that you check the plan specifically for maximum rates and annual limits if you have chronic needs. This is where good clauses for therapeutic remedies separate from mediocre ones.

Do I need a new prescription for each series?

Yes, the prescription is the basis of every reimbursement. The doctor issues follow-up prescriptions if further therapy is medically justified. The plan does not reimburse treatment without a valid prescription.

What applies to physiotherapy after an accident at work?

After accidents at work and for occupational diseases, the statutory accident insurance is the primary payer, not private health insurance. The practice then bills the employers' liability insurance association directly. For private accidents, the normal reimbursement process applies.

Does the plan also cover prevention courses such as back training?

That is prevention, not medical treatment. Some plans pay a subsidy toward courses, most do not. Exercise therapy prescribed for existing complaints is separate from this and is reimbursable as normal.

Switching to private health insurance is probably not advisable for you.

From the age of 55, admission to private health insurance is often possible only with restrictions. A late switch can also mean higher premiums.

It matters to us that a switch pays off for you in the long run. That is why we do not recommend a consultation when your answers already show that the conditions are rather unfavorable.
