Frequently asked questions about the effective premium

What is the difference between the gross premium and the effective premium in private health insurance?
Our calculator shows exactly this difference. The gross premium is the amount you transfer to your insurer every month. The effective premium subtracts the tax benefit: as a self-employed person, you deduct the basic cover share of your premium as a special expense. The tax office refunds part of it through your tax return. Depending on your marginal tax rate, this lowers the real premium by 30 to 42 percent of the tax-deductible share.
How does the calculator take the employer contribution for employees into account?
Employees receive an employer contribution to private health insurance. It is 50 percent of the notional maximum contribution to statutory health insurance, up to €508.59 a month (as of 2026). The employer contribution is tax-free, but it is offset against your special expenses. The calculator subtracts it automatically and shows your actual own share.
Why is the effective statutory contribution in the table higher than the amount on your fund statement?
The calculator works out the statutory contribution from your income up to the Beitragsbemessungsgrenze of €5,812.50 a month. If you earn more, you pay the maximum contribution. On top comes the long-term care contribution, which depends on the number of children. The basic contribution share of statutory health insurance is also tax-deductible, but the employer contribution is offset against it.
