Reserves first, prepayment second
We recommend a prepayment only if at least three months of gross spending remain as a free reserve after the payment. Otherwise a cash shortage eats up the discount.

How does the consultation work? →A premium prepayment is an individual agreement with the insurer. Instead of paying monthly, you transfer the premium for one year or several years in advance, documented in the insurance policy. For annual payment, a discount of two to four percent is common. Some insurers pay only one percent, and some large providers give no discount at all. For prepayments over several years, higher rates are possible in some cases.
Which rate applies is set out in the plan terms, and you should ask for it in writing before you decide. In business terms, the discount is a risk-free return on tied-up capital. Whether it is attractive depends on what the money would earn elsewhere. That is exactly why the opportunity question belongs in every prepayment decision, not only the discount rate.
A discount on the annual premium is quick to calculate (guide values):
The figures show both sides. The saving is real but modest. An annual prepayment only shows its real value together with the tax lever.
With an annual prepayment, tax works as a second lever. Basic contributions to health and long-term care insurance are fully deductible under § 10 para. 1 no. 3 EStG. For future premium years, the tax office accepts prepayments up to three times the annual premium owed under the contract, in addition to the current year (sentence 5, and until 2019 the limit was two and a half times). The trick lies in the following years. Because no health insurance premiums are due there any more, the other provision ceiling under § 10 para. 4 EStG becomes free for liability, accident or Krankentagegeld KrankentagegeldPrivate sick pay: a daily allowance from private insurance while you are unable to work. It replaces lost income from an agreed day of illness.More about Krankentagegeld → premiums that would otherwise have no tax effect. That ceiling is €2,800 per year for people who pay their own insurance, €1,900 for employees and civil servants, and the sum of both ceilings for joint assessment.
An example. If you pay an annual premium of €6,000, without bundling you deduct €24,000 of health insurance premiums over four years, and your other provision expenses largely have no effect. With a prepayment of three future annual premiums in the first year, this becomes up to €32,400. The same €24,000 is bundled in the first year, plus the freed ceiling of €2,800 in each of the three following years. At a high marginal tax rate, this creates a saving in the four-digit range, in addition to the discount.
In practice, the calendar decides first. The payment must arrive by December 21. Otherwise the ten-day rule of § 11 EStG applies and the deduction slips into the following year. The insurer should confirm the prepayment in writing, with the amounts split by insurance year, because the tax office wants to see exactly this certificate. Agree on the specific arrangement with your tax advisor in advance. This overview does not replace tax advice.
Clarify three scenarios before you pay. In a premium adjustment, the new premium usually also applies to prepaid periods. The insurer offsets the difference or asks for an extra payment. Only some insurers guarantee the premium for the prepaid period, and then this is stated explicitly in the agreement. In an internal plan change, the prepaid amount is converted to the new premium.
If the contract ends, for example through a change of insurer or a return to the statutory health insurance fund, the insurer refunds the unused share. The discount granted for periods that were not reached is usually deducted again. If you are considering a switch in the foreseeable future, you should therefore not agree on a prepayment over several years. The same applies to employees whose salary could soon fall below the Versicherungspflichtgrenze.
The question of whether an annual prepayment pays off almost always comes from self-employed people, and our answer is nuanced. As a pure discount deal it is solid but unspectacular. As a tax instrument in the right year it can move four-digit amounts.
We recommend that self-employed people with good cash flow use the prepayment not as a pure discount tool but combined with targeted tax bundling in a high-income year. Civil servants and retired civil servants are the second group for whom the calculation often pays off. Their privately insured share of the remaining costs is modest, the ceiling of €1,900 is rarely used up, and bundling creates the same room there. For employees, on the other hand, the calculation usually tips the other way. The employer contribution keeps flowing monthly, but in years without your own premium payment it becomes taxable and largely eats up the advantage. It can only pay off there with very high premiums and only after an individual calculation.
For tax purposes, three times the annual premium is recognized in the year of payment. Under the contract, many insurers allow prepayments of this size. Each insurer sets the exact upper limits and discount scales itself.
The employer contribution remains a monthly item in the payroll and is based on the monthly premium owed. A prepayment does not move it forward, and this makes the model less attractive in administrative terms for employees than for the self-employed.
The contract ends, and the unused share of the premium is refunded to the heirs. So the prepayment is not lost money, but it should be known in estate planning as a claim against the insurer.

From the age of 55, admission to private health insurance is often possible only with restrictions. A late switch can also mean higher premiums.
It matters to us that a switch pays off for you in the long run. That is why we do not recommend a consultation when your answers already show that the conditions are rather unfavorable.
