Continentale combination for the self-employed with free choice of doctor, high cover for alternative practitioners and a no-claims refund of up to six monthly premiums, balanced by a deductible per benefit.
Data as ofJuly 2026, source: plan terms and our advisory practice
Why this plan is in our selection
The Continentale combination of Comfort-U and SP2 is in our selection for the self-employed because it combines free choice of doctor with strong reimbursement for naturopathy and a high no-claims refund. The structure has two parts. Comfort-U covers outpatient and dental benefits as well as basic inpatient cover, and SP2 raises hospital cover to a two-bed room and private doctor treatment. Instead of a fixed annual deductible, the plan works with a deductible per benefit, usually €20. We rate the benefit level, family friendliness and claims handling at 7 out of 10 points each, and the premium level also at 7 out of 10 points. We have to be honest about premium stability: at 4 out of 10 points it is the weakest value in this comparison group. If you decide on this plan, you should factor in the possibility of larger future adjustments from the start.
01
Free choice of doctor without restrictions
The plan has no primary doctor principle. You choose doctors and specialists freely, without having to arrange your first treatment through a family doctor. Reimbursement for alternative practitioners is particularly strong: treatment according to the fee directory is covered at 100 percent.
02
High no-claims refund
If you submit no bills, you get a lot back. The plan combines a guaranteed lump-sum payment of two monthly premiums after the first claim-free year with a performance-based refund that reaches four monthly premiums after five years, together up to six monthly premiums.
03
Also calculated as a plan for doctors
The same plan line is also offered to medical professionals on favorable terms. For the self-employed, this points to a broadly calculated plan with a clear benefit profile.

Strengths and limits
No plan is the right choice for everyone. This is what speaks for this combination, and this is what you should know.
What speaks for it
✓
Free choice of doctor without a primary doctor principle, plus free choice of hospital with a two-bed room and private doctor through SP2.
✓
Very strong naturopathy: alternative practitioners at 100 percent according to the fee directory, homeopathic medicines included.
✓
High no-claims refund of up to six monthly premiums together for claim-free years.
✓
Fertility treatment at 100 percent after approval, dental treatment including professional teeth cleaning at 100 percent.
✓
Deductible per benefit instead of a fixed annual deductible, so your share is predictable in each case.
What you should know
!
We rate premium stability at 4 out of 10 points, the weakest value in the comparison group. You should factor in larger future adjustments.
!
The deductible applies to every benefit. If you have many small bills, this adds up to noticeable amounts of your own.
!
A need for visual aids that already exists when the contract is signed is only covered against a surcharge, and classic health cures are not reimbursed.
!
Children cannot be insured alone in this plan, and there is no option right for higher-level cover.
!
A plan with a good benefit profile only pays off if your application is accepted cleanly. If you have pre-existing conditions, we recommend an anonymous risk enquiry before you apply.
Who this plan suits
The combination suits the self-employed and freelancers who value free choice of doctor, strong naturopathy and a high refund and are willing to accept a deductible per benefit and a weaker premium history for it.
It is less suitable if maximum premium stability, a single room or an option right matter to you, or if you regularly submit many small bills. For these cases we show you better alternatives from our comparison in our advice.
Premium development at Continentale: A plan is only as good as its calculation. We show the adjustments of past years transparently by year and plan.
Instead of a fixed annual deductible, Continentale deducts a small share of your own for most benefits, usually at most €20 per day or product, up to €50 for medical aids and €20 per day for at most 14 days in hospital. Generics are excluded. The advantage is a moderate premium, the disadvantage adds up with many small bills.
Can I deduct my premiums from tax as a self-employed person?
⌄
You can claim the basic share of your premium, meaning cover at the level of statutory basic care, as pension and insurance expenses for tax purposes. We explain the details and maximum amounts under deducting private health insurance from tax.
Can I adjust the combination later?
⌄
Within Continentale you can switch to similar plans using the right to change plan under § 204 VVG, and you keep your ageing reserves. However, the combination has neither a simplified right to switch to higher-level plans nor an option right.
What happens if premiums rise?
⌄
Premium adjustments are part of how private health insurance works. What matters is their long-term size. You can find Continentale's specific adjustments by year and plan in our overview of premium adjustments. If an increase is large, we check plan optimization with you.
How is our rating calculated?

Switching to private health insurance is probably not advisable for you.
From the age of 55, admission to private health insurance is often possible only with restrictions. A late switch can also mean higher premiums.
It matters to us that a switch pays off for you in the long run. That is why we do not recommend a consultation when your answers already show that the conditions are rather unfavorable.
