Strong AXA combination for employees with reimbursement above the maximum rates of the fee schedule, a flexibly selectable deductible and a separate dental module.
Data as ofJuly 2026, source: plan terms and our advisory practice
Why this plan is in our selection
The AXA combination of GesundExtra and Premium Zahn-U is in our selection for employees because it combines a broad range of benefits with unusual flexibility. The cover consists of two modules. GesundExtra covers outpatient and inpatient care, and Premium Zahn-U adds dental benefits as a separate dental module. Both also reimburse above the maximum rates of the fee schedule, and that is exactly where many specialists bill. A special feature is the right to change the deductible within the same plan on January 1 of each year, without having to renegotiate the scope of benefits. We rate benefit coverage at 8 out of 10 points and claims handling also at 8 out of 10. The premium is in the upper middle range, so we give 4 out of 10 points for the premium level. If you choose this combination, you pay for a solid package of benefits with room to move on the deductible.
01
Deductible can be changed flexibly
The amount of the deductible can be adjusted within the same plan on January 1 of each year, without the scope of benefits being checked again. This lets you adapt your share to your life situation instead of committing once. The sample cover includes no deductible.
02
Dental cover as a separate module
Premium Zahn-U is a separate dental module that you choose independently of the main cover. It reimburses dental treatment at 100 percent and dentures, inlays and implants including bone grafting at 90 percent, and also pays above the maximum rates of the fee schedule for dentists.
03
Guaranteed lump-sum payment
For a claim-free calendar year, GesundExtra pays a guaranteed lump sum of €900, pro rata per month if the policy starts during the year. Preventive checkups, vaccinations and prevention courses do not put this entitlement at risk.

What you should know
!
Dentures, inlays and implants are reimbursed at 90%, not at 100%. For larger dental work a share of your own remains.
!
The dental benefit limits cap dental benefits in the first four years at €2,000 to €8,000. If you plan larger dental work soon, you should factor this in.
!
The combination has no simplified right to change plan. The statutory right to switch under § 204 VVG of course still applies.
!
Krankentagegeld must be added separately and belongs in every calculation for employees.
!
The premium is in the upper middle range (4 out of 10 points for premium level). If you have pre-existing conditions, we recommend an anonymous risk enquiry before you apply.
Who this plan suits
The combination suits employees above the JAEGJAEG · JahresarbeitsentgeltgrenzeThe annual income threshold in Germany. Employees who earn more than this per year may leave statutory health insurance and switch to private health insurance.More about JAEG → who want a broad package of benefits and value room to move on the deductible. It is also a good choice for families, from the covered companion to fertility treatment.
It is less suitable if the monthly premium is the deciding criterion or if dentures should be reimbursed at 100 percent instead of 90 percent. For these cases we show you better alternatives from our comparison in our advice.

What does the combination of GesundExtra and Premium Zahn-U consist of?
⌄
GesundExtra is the main cover for outpatient and inpatient benefits. Premium Zahn-U is added as a separate dental module and covers dental treatment, dentures and orthodontics. Both modules can be looked at independently, so dental cover can be planned in a targeted way.
Does my employer contribute to this plan?
⌄
Yes. As a privately insured employee you receive the employer contribution of half your premium, up to the statutory maximum. The contribution applies no matter which plan you choose.
Can I change the deductible later?
⌄
Yes. The amount of the deductible can be adjusted within the same plan on January 1 of each year, without new health questions, and an application in advance is required. For switching to other plans, the right to change plan under § 204 VVG also applies, and you keep your ageing reserves.
What happens if premiums rise?
⌄
Premium adjustments are part of how private health insurance works. What matters is their long-term size. You can find AXA's specific adjustments by year and plan in our overview of premium adjustments. If an increase is large, we check plan optimization with you.
How is our rating calculated?
⌄
The five key figures come from our ongoing plan analysis. They are based on the plan terms, the published premium histories and our experience with claims handling from advisory practice. The rating is our professional assessment, not a ranking by a testing institute.

Switching to private health insurance is probably not advisable for you.
From the age of 55, admission to private health insurance is often possible only with restrictions. A late switch can also mean higher premiums.
It matters to us that a switch pays off for you in the long run. That is why we do not recommend a consultation when your answers already show that the conditions are rather unfavorable.
